Would one still need to pay the subscription for the base VaM software on Patreon in order to use this? I'm confused on the logistics

No. You are paying for VaM and vamX together. You do end up paying MeshedVR (VaM on Patreon) when buying this, just through itch.io. It comes with permanent free updates for VaM (the current VaM, also knowns as VaM1)
