How does someone get there? Any hints?

It's directly South of Sunrise Village or South-South East of Eastern Checkpoint.

You probably mean that island, where Lona can end up if washed away into sea?
Find any shore on the map, and go across it, for as much steps as needed.
Big wave would appear, don't resist and let it wash you away into sea... There are random chances to end up on any other shore on the map, including that island which is naturally off limits... That's quite an adventure btw. ๐Ÿ˜˜
Spoiler alert:
There's a previous ship that went off the Noer island with refugees, which crashed on that island leaving everyone stranded...
Same ship carried seeds of parasitic flower ๐ŸŒบ(aphrodesiac drug), and whole island ended up sprawling with this plant... It also being the dominating food source there, except for few fish and bananas.
So you can figure out what the stranded crew ended up doing, and what would happen to Lona. ๐Ÿ™‚
Also, better leave your stuff at Tripartite Exchange bank, because each time you're washed away into sea โ€“ a lot of your gear is lost.
Also, it's possible to make a good bit of money by collecting as many parasitic flowers ๐ŸŒบ while at the island , and then selling 100s of them upon return to Noer.

I haven't stuck around long enough for a tidal wave from the shore, mainly because of storyline Quests and RolePlay-S.

5 Tidal Waves at 17 World Difficulty + Hitchhiker Trait... no dice.

Either I drop the Hitchhiker Trait, or Raise the World Difficulty by sleeping a lot + having lots of food.

world difficulty doesn't affect wave nor does hitchhiker trait its just very rare and not gaurenteed to bring you to the island and hitchhiker actually doesn't reduce bad encounters rather it doubles your chance of a neutral encounter

Goodness Gracious... This feels like astronomy, waiting for the stars to align, kind of like a Full Moon for once a month.

It's been 30 days in-game, waiting on a stake out with so much Patrols, Bandits, Goblins, Refugees, and Abandoned Carts. ๐Ÿ˜…

You know, I have suspicion that the event could be not entirely random, but instead hardcoded into the game...

A few times I had the first wave in playthrough โ€“ immediately taking me to that island...
But if I skipped that 1'st wave โ€“ I couldn't get to the island after dozens of more tries... ๐Ÿ™„

Dunno, only the gamedev would know for sure, but it's just my hypothesis โ€“ that there could be some scripting involved.
I have a new game save still around, and can double-check that...

But in any case, that 3'rd island is so obscure, I didn't know if even existed for like good 4 years replaying this game time after time, since 2021.
So, if you'll get there, you'd be among the elite. ๐Ÿ‘ง๐Ÿป๐Ÿ†

they are very random actually but i could never figure out the rng of vx ace

appearntly from what i looked up it uses ruby's implimentation of RNG which is just a Mersenne Twister pseudorandom number generator and the rng seed is gathered at game boot from system entropy  apparently in effect unless you override the starting seed you will not get the same playthrough every time since the amount of permutations that Mersenne Twister goes through would take longer than your entire lifetime to roll over
