Most often, as a Bad Thing that can happen as a low double, or as a result of failing a roll to *avoid* taking damage.  If something happens in the story that would be likely to cause damage, I usually call for a roll to avoid, resist, or bounce back from it, with difficulty based on how dangerous the cause would be.
