I enjoyed the demo! Since ya asked for feedback:
-Lily and Ruby's dynamic is really fun, and ABDL games written from the perspective of the caregiver are super rare, so this was a treat.
-The story so far is very cute! It was fun getting to coddle Ruby, and I'm looking forward to meeting (and babying) the rest of the cast, too.
-The options menu is really cool! If you're open to suggestions, it would be neat to see some other ageplay-related options, like giving the little a rattle or blocks to play with, asking if they need to go potty, reading a storybook to them, putting them in the crib for a nap, giving them a snack in a highchair, or playing different childish games with them like tag, dress-up, house, doctor/checkup, or hide-and-seek. Some consensual spanking content might be fun too. Since there's a potty chart, pullups might also be a fun option for some customers.
-The diaper content is excellent, especially the wetting. The messing is a bit too abrupt, though and a tad more squirming and desperation would be cute to see. ;3 A second view of Ruby from the back would also be nice, to show off that cute padded butt and more of the sag.
-I like the art! The game's clearly made with love, and has its own style. Ruby's character design is adorable, and I love her little shortall dress with the baby bottle logo on the shoulder. The linework and proportions are a bit loose/stylized, but it works well in context. My only critique would be that the arms on Ruby's sprite are a bit big relative to her body. But that does make her look more doll-like, so it does make sense as a stylistic choice.
-The background art is cute, too! There are a bunch of fun little details like the post-it-notes under the counter, and the potty chart.
-When Lily is done changing Ruby (or another employee or customer), maybe add a second menu with options for aftercare? Like, a hug or some cuddling. It's important to make your little feel appreciated after the teasing is over. ;p
-Consider adding a download button to the game page, or cross-posting the game elsewhere, just in case Itch de-lists adult games again. I think it would be well received on the omorashi org site and maybe on diapered anime, among other places.
-Caught a small typo in the opening passage: "But despite the businesses looks" should probably be "But despite the business's looks".
-I like how choosing different options reveal different parts of Lily's backstory and what her co-workers are like.
-The overall design and premise are suuuuuper cute. I especially like the idea of helping shy guests explore diapers and other embarrassing, babyish stuff in a safe and comforting setting. (Shy littles are awesome!)
