Visited Ethexae early in the game. Unlocked her personal quest. Decided to do it later. Came back to Myrkkan I on turn 24 for Agonaglia. Passed the Endurance check, but not the Sprituality check. Talked to Ethexae, didn't get her approval. Went to Heru III on Turn 26. Talked to Radomil. Game says I lost the Starfall game.
But I never played the Starfall game, so that doesn't make much sense.

My "Princess Errant" Steam achievement is not tracking the personal quests that I've completed. I've done at least three, though I've yet to get any ending other than death. Not sure if this is a bug or is actually working as intended, but I lean bug since the "Dead Princess Storage" achievement is tracking how many different death methods I've experienced.
I'm unsure if this is a bug or just some very complicated math happening behind the scenes, but this came up during turn 27:

This seems to be the only place I found to report bugs, minor as they may be
1) In the quest "A personal Journey", skill checks for "Environment" are incorrectly labelled as "Terrain"
2) During that quest, failing an Endurance [Student] skill check applies the "Fatigued" debuff. Whether this is a harmless display bug or a more permanent one, the debuff remains for the rest of the campaign.
Terrain is a skill. It is one of the three skills under the Environment group. Those checks are specifically Terrain checks.
When you say "the rest of the campaign" do you mean it doesn't go away after the end of the quest? (Just clarifying because I'm not sure precisely what you mean by 'campaign'.)
Oh wow, you are correct about the Terrain skill, I completely missed that.
And about the "Fatigued" debuff, I mean it lasted forever, all the way until turn 35. Whether it truly impacted my skill checks, I don't know, but my skill screen clearly showed those "double bars", with the top one being my normal skill and the bottom one missing 10 points in red.

During your pilgrimage to the Blind Eye, if you manage to keep your mood other than Compliant, you might get the opportunity to lie.
My check for "Sleight-of-Hand [Practiced]" was a Fail, but "Deception [Practiced]" was a success.
You get some text describing that maybe no one ever sees anything here, however before you get the choice to "Pretend you had a vision", I had to click through a completely empty text box: 
I believe I spotted a bug in the Mau Electoral Reception; the Sight Enhancers don't seem to do anything.
All other things being exactly the same (day 20, same choices in daily events, same choice in elector discussions, no training in relevant skill checks to modify approval), the googles don't change the elector's approval at all. In fact, the results are exactly the same as selecting "Bright Reception". This is true whether or not you pass the Repair + Visual Arts [Practiced] check to get the stylish or silly Sight Enhancers, respectively.
not sure if this is a bug, but some advisor bonuses do apply while "meeting Death" with Krovos, but others don't; maybe that's just because the timing of things - like, when you're alone in the dark you don't have any way to get assistance from your logothete?

I've been saving up issues, so this post will be more loaded than usual.
1) During Sinniparrt's quest "Signs in the ice", there is no pop-up confirmation that the OPERATIONS [GRADUATE] test for "Scan for lost fragments" is successful.
2) Typo: On turn 21, if you succeed on the test to "Send medical teams" (Science + Bureaucracy [Student]), after your training session, the summary will incorrectly reference Heru II instead of Heru III

3) Suggestion: The trap room at the end of Radomil's Obstacle course makes me believe that death should be by stabbing, instead of concussion.
4) Typo: in Eve's quest "a personal touch", if you make hair jewellery and succeed on the engineering checks, you set your creation in delicate metal "filgree". I believe the proper term is "filigree".

5) I saved the best for last: I found an actual, reproducible crash. If you try to run a Imperial Pool + (using skill Media 90) immediately after adopting a baby on turn 8 (no matter what name you pick), you'll get the crash. If you take any other social action in-between (such as a security alert from Korundi), the crash won't happen.
