Awesome, great to see a monster get a whole entry! a fun weird horror to throw into a game, especially a dying-earth, weird and gonzo one. (Could also slot into basically anything, Mork Borg would be a good pairing too.)
Your layout is really clean and clear, with great presentation: the cover is great, font works well and the layout and graphic design is top notch) The only faults I can see are some typos, so another run of editing would do it some good.
The monster is well-thought out, in the sense that it doesn't require too much thought: percentages of player health for damage and nigh-invulnerability make it easily scaleable, but also clear that it is more a vehicle to generate shenanigans. You may have been able to add a larger table for the demands, which would have been nice since you have plenty of space, but I think that would be harder to do and the presentation is already great.
