so you have to fatten a girl (or yourself) to the enormous status. You then need to make sure to have a bariatric bed or the reinforced floor so you don't get shipped to the care center. You should get a message from the obesity clinic to start a new side quest where you can earn tokens to buy the gantry. you can always travel to the hospital to see if the clinic is open to you in case you don't get the message.
