I also think it would be good to add an Ending Monologue if the game has finished, and the floors have been completed.

If there isn’t one already, the storyline could possibly imply that Patient Zero was really the one creating everything, the monsters and such (in some way)— but had only remembered after escaping the place, and understands it fully.

It could be something like this, similar to the first beginning dialogue:

“When things stay in one place for too long, they perish, they rot.

And so I, Patient Zero— who had once been locked under inevitable stillness— have deteriorated, entirely.

The core of the hospital I must be, holding up what remains of its legacy…

I have been there since the start, that is who I am, now.

So as Patient Zero, the very foundation of the place… I had formed everything, torn it apart at the seams, and taken my leave.”

Here is the dialogue for Patient Zero. I have grouped together two lines to be shown (at some point) on each floor. The dialogue from top to bottom represents their thoughts of the first floor to the last.

[Also, I think the title of the game should be called “Ward 666: Patient Zero” unless there are any other combinations or words you preferred to use.]

“I wonder if anybody else roams this floor…”
“The lift will work out nicely— I’d do anything to leave.”

“What sort of… elaborate scheme is this?”
“I need to stay low; any one of those beasts could be around the corner.”

“I can’t imagine what must have happened here.”
“This is hellish, yet I still breathe, I live. Is this purgatory?”

“If only there weren’t so many floors…”
“This feels like forever.”

“…It seems like I’m the only one here. The first to enter, last to leave.”
“Why have I been so blessed? I wake, and live another day.”

“I often question how I’m not dead yet.”
“…”

[The screen starts black, words in white text will appear and fade in and out as the player proceeds to see each line of dialogue.]

Darkness.

That has been the velvet over my eyes for a very long time.

So how does the world expect me to open my eyes?

To open my eyes, into harsh fluorescent lights.

--pause--

I have no memory of my true identity. Even lesser, my name.

But Patient Zero was the last of things that were whispered to me.

Whispered during my state of nothingness, and state of sleep...

So Patient Zero is what I should go as, and leave.

--black screen fades out, game starts--

[The mentions of Patient Zero would be in red text, the rest in white.]

I think there should be a simple, introductory monologue at the very beginning of the game (for the first playthrough), which will give the players a brief idea of the storyline whilst keeping the atmosphere of mystery.

I have written an example which I think could work if this feature is going to be implemented.
