I gotta say, I laughed out loud when AM was part of the background visuals. The editing you've done does a fantastic job at making everything feel as chaotic as it should be, just like EM is feeling.
The parts where he's walking around, wanting to be nothing, contemplating whether he wants to live were poetic to me. I had to stop and appreciate them for a few seconds before moving forward.

Honestly, out of all I've read so far, nothing has had me squirming in my seat so much as reading someone perform surgery when they know they don't know what they're doing. Hhhhhooboy. You did great! Favorite visual detail is the way you did Summertime's eyes.

I found this both parts horrifying and charming. I think the humor does a great job at adding to the surreal nature the writing! I especially loved the tension at the end of the black route, too. The music does a lot to cushion the horror in the first two routes, so having it cut out made it feel more "real", in a way. Banger song choice, too.
I'm glad he's relatable!! So, funny thing is, *all* of the characters are based on other fictional characters that I headcanon as transmasc... Otto is based on Otacon from Metal Gear Solid, but fused with Alphys from undertale, and I knew in my heart of hearts he'd be a fujoshi to the core from that, heheh.

The Little Mermaid was one of my favorite movies as a kid. It's...Kind of a trans movie, in its own way. But that aside, this art is so gorgeous, so professional. And knowing about clownfish protandry makes the worldbuilding feel very clever, too.

The mixed media art style is fascinating to me... I feel like it does well to convey the nightmarish nature of Ian's situation. And it is nightmarish... it hits close to home in a lot of ways.

This is a really solid start! You've done a good job at making Jared suck and making Mackenzie empathetic... The parenting-writing is very on point, too. I loved the transformation sequence, and my favorite scene is the ren faire... Makes me miss attending.
