For the Irene bug, yes its the one where she gets on the floor and it happens afterward. Since 3.4 I haven't been able to get her to save me at all. I think another trigger for this bug was just going to the Shepherd household in general with Irene alongside you? Perhaps the charm that cancels her out from helping also keeps her bugged?
As for the second bug I listed, I played on the English/First Language set so yes. For who triggers it, I believe it was any of the new npcs, including the test ghosts? I know the MS. K ghost triggers it after someone named Hiromi gives dialogue, as well as Mason's new dialogue with the new person who appears in the asylum. I can't test Rachel, Tori, or any of the other quest locked npcs as I'm not sure how to start the new quests at the moment, so for now I'd assume they are the same.
As for a possible third bug(?) It says I own both Tori and Rachel without even meeting them yet in game.
