I think of a suggestion to avoid Minor controversity, the suggestion is to add a minimum age of all NPCs and customization to be atleast 18+ and a maximum/cap that doesn't let them become old, same would apply to player, each 360 days you and all npcs would age +1, but they stop aging when they reach the max/cap.

Maybe wrinkles on the face or body that would dissapear as the characters get fatter and maybe a saggier fat deposition, different from body type.
